GOSHEN — A $50,000 grant will help those in the community with chronic diseases.

CVS Caremark Charitable Trust awarded the grant to Maple City Health Care Center April 25. The grant will be used to provide prevention information to people who are at risk to develop diabetes and to help people with diabetes, partially through Maple City Health Care Center’s Healthy Living groups.

Maple City Health Care formed Healthy Living groups so patients can help each other set goals for change and hold each other accountable. These groups empower members to make positive health changes with the help of their peers.

The next Healthy Living group will begin meeting Thursday, May 2.
