INDIANAPOLIS — An Elkhart facility and one in South Bend received recalled medication linked to an outbreak of fungal meningitis.

State health officials said the OSMC Outpatient Surgery Center in Elkhart and the South Bend Clinic received batches of back pain medication recently recalled by Framingham, Mass.-based New England Compounding Center, which has been linked to a widening outbreak that’s sickened dozens of people in six states. Five deaths have been reported.

The State Department of Health said Thursday it is investigating the confirmed Indiana case of the illness as well as “other potential cases.”

Four other Indiana facilities that received the medication are: Ambulatory Care Center LLP in Evansville; the Fort Wayne Physical Medicine; Union Hospital in Terre Haute and Wellspring in Columbus.
